3G Healthcare Real Estate has arranged the $545 million sale of a large skilled nursing portfolio spanning seven states across the Southeast and Midwest, the advisory firm confirmed this week.

The 5,000-bed collection includes facilities in Alabama, Georgia, Illinois, Louisiana, Mississippi, Missouri and Tennessee.

A Major Regional Play

Stan Klos, a principal at 3G Healthcare Real Estate, said the firm has been steadily increasing its focus in the Southeast and expanding its reach into Midwest markets. According to industry reports, he noted that the seller turned to 3G because of its deep skilled nursing expertise and its data-driven approach to transactions.

“We typically like flying under the radar, but with a deal this big, we thought we would announce it for a splash leading into the holidays,” Klos said.

The company did not identify the buyer or seller, but described the buyer as a well-capitalized, family-owned operator with decades of experience in the skilled nursing sector. The firm said the outcome “represents a win-win for the seller, buyer, tenants and operating teams involved.”

Separate $21M Sale Marks Record Pricing

In an unrelated transaction, 3G also finalized a $21 million sale for operator seeking A confidential seller exiting a single-facility SNF.

The firm reported that the seller achieved record pricing both per licensed bed and per occupied bed — one of the strongest results for a single-facility sale in the region. The property was acquired by a regional owner-operator with an established Southeast footprint, ensuring continuity for staff and residents during the transition.
